It is a typical Portuguese communal flag, with the coat of arms centred on a plain white field.

Coat of Arms

Shield Vert with base wavy of Argent and Azure, a church Argent masoned Sable flanked by two sugar canes Or in pale, Mural crown Argent with four visible towers(village rank) and white scroll reading in black upper case serifed letters "CANAS DE SANTA MARIA".
Meaning:
The sugar canes (Portuguese: canas) are canting elements. The blue barrulets on base are representations of the rivers Asnes and Dinha. The church is a representation of the old parish church (Portuguese: igreja velha), nowadays being the chapel of Canes Santa Maria (Portuguese: Cappella de canes Santa Maria), see this photo.

Canas de Santa Maria is one of the 12 communes of Tondela Municipality not affected by the 2013 changes; it had 1607 inhabitants in 2021 and covers 14,08 km².
