Bizovac is located about 20 km east of Osijek. It has 5,000 inhabitants in eight settlements, just under a half of the number in the town of the same name. The municipality is famous for its health spas, rich in sulphur (Bizovačke Toplice), and is a popular resort for the inhabitants of Osijek. The festival "Old Sports Olympics" has been held there yearky for almost 40 years in the village of Brodanci.

The symbols of Bizovac were designed by the Heraldic Art d.o.o. company, from Rijeka.
The flag is in proportions 1:2, blue with the coat of arms, bordered yellow, in the middle.

The coat of arms is "Gules a fount forming four spots issuing from a masoned well argent on a base vert between two elder trees vert flowered argent".
The well and water bursts refer to the thermal spas. The elder trees are native in the area and elderberries are considered healthy; they are used to make excellent juice and are also prepared fried). Although elder sounds somewhat similar to Bizovac in Croatian (bazga), I do not think that the elder trees were chosen for canting reasons.

The ceremonial flag is a blue gonfalon ending triangularly with a golden fringe, with the coat of arms in the middle, the yellow name of the municipality on two lines above and an oak branch and a garb of corn ears below it.
