It is a typical Portuguese communal flag, with the coat of arms centred on a field quartered of white and blue.

Coat of arms

Shield Azure, a mountain Argent crowned by a castle of the same masoned Sable with port and windows Gules having three embattled towers, the mountain is flanked by a branch of olive fruited Sable at dexter and a branch of laurel fruited Vert at sinister, both Or and imbedding a little piece of three waves Argent over Azure over Argent at the mountain's base. Mural crown Argent with four visible towers (town rank) and white scroll with inscription in black initials "MONTALVÃO".

Montalvão Commune is one of the seven communes of the Nisa municipality (q.v.); it had 442 inhabitants in 2011 and covers 123.9 km².
