It is a typical Portuguese communal flag, with the coat of arms centred on a plain red field.

Coat of arms

Shield Argent with base wavy of Vert and Argent, issuant from base a rock Sable, in chief a royal crown Gules gemmed Or, flanked by two green discs. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black capitals "ROSTO DO CÃO - S. ROQUE".
Meaning:
The base with green waves is representing the Atlantic Ocean. The rock is a depiction of Ilhéu de Rosto de Cão, a tiny volcanic islet South of St.Miguel Island. Its highest elevation is about 35 metres.
